Please fill in the necessary information, attach documents for identity verification and the fee, and send it to the "Personal Information Inquiry Desk."

Documents used for identity verification must be valid at the time of receipt by our company or must have been created within the last three months. ◯ Fees and collection methods

A fixed-amount postal money order of 1,000 yen will be required for each request for disclosure, notification of purpose of use, etc. (fees for issuing the fixed-amount postal money order will be borne by the individual).
This fee includes the registered mail fee when the company sends the document to you.
If any other costs are incurred, they will be charged separately.
There is no fee for correcting, adding, deleting, or suspending use of content.
Fees will be collected by fixed-amount postal money order.
